number in the above entitled case.  Thank you.

May I please impose upon you to provide us with a written receipt
for the  docket fee?  You did not tell us whether or not the $100
USPMO was sufficient to cover the full fee.

Also, is  our caption  correct on the original petition?  If not,
please provide us with a sample of the correct court caption.  We
prefer our  form to  be proper,  out of  respect  for  the  state
judiciary.
