This is a lawful request under the Freedom of Information Act (“FOIA”), 5 U.S.C. 552 et seq.

Please provide me with a certified copy of the following document:

1. Form 3801 signed and dated by the Grand Jury Foreperson, U.S. District Court, 280 S. First Street, San Jose, California, authorizing one Susie Bauer [sic] to receive registered U.S. Mail on behalf of said Grand Jury Foreperson [see attached Forms 3806 and 3811, line C.]

Please take careful note of the fact that our Registered U.S. Mail, serial number #RR-507-445-419-US, shows a delivery address that is different from item 1, but the actual delivery address was not completed on Form 3811 (see line D.)

I request a waiver of all fees for this request.

Disclosure of the requested information to Me is in the public interest, because it is likely to contribute significantly to public understanding of the operations and activities of the federal government, and is not primarily in My commercial interest.
