Subject:  Freedom of Information Act (“FOIA”) Request

 

Dear Disclosure Officer:

 

This is a request under the Freedom of Information Act (“FOIA”).  I hereby request that a certified copy of the following documents be provided to Me, without delay:

 

1.               latest Form 10-K Annual Reports, pursuant to sections 13 and 15(d) of the Securities Exchange Act of 1934, for “any corporation in which the United States of America” [sic] is, or ever was, a stockholder, as the term “United States of America” is used in the Revision Notes for Title 18, United States Code, secs. 286 and 287 (see Oct. 23, 1918, ch. 194, 40 Stat. 1015), and as that term is likewise used at 28 CFR secs. 0.64-1 and 0.96b [see attached].

 

I agree to pay up to $25.00 for search and review fees.  If the documents I have requested require higher fees, please inform me in writing first, so that I can make necessary arrangements to pay those higher fees.

 

Disclosure of the requested information to Me is in the public interest, because it is likely to contribute significantly to public understanding of the operations and activities of the federal government, and is not primarily in My commercial interest.
