Would you  kindly provide  me with  the  proper  Nicholas  County
Recorder's form  for filing  and recording a Lien for Labor, also
known as a "Mechanic's Lien," in your records?

I have  attached the  comparable form  used in Arizona state, for
comparison purposes.

If your  office does  not provide  such a  form, would  you  then
please provide  me with  directions to  one, or  more, stationery
suppliers of the correct form for California state?
