standard form says that you are free
to modify this form (one of the few
which CAN be modified). Ask your
bank if they have any in stock.
I presently don't have any of the
standard ones which I can fax to you,
but I can email to you the ASCII version
of the modified one we developed several
years ago, just as the research about
Title 27 was beginning to surface.
