Question on IRS form 2678

Hello Paul,

I have been following this board for several months and enjoy
reading the posts and your knowledge of income taxes.

I just recently (about 8 weeks ago) had my employer stop holding
Federal Income taxes. They would not stop withholding on Social
Security, medicare, state taxes, and local taxes. I am still working
on that issure
Today I received from the IRS a Paperwork Reduction Act Notice.
They want me to provide them information on IRS form 2678, Employer
Appointment of Agent. In all my reading on the income tax fraud, I
don't believe that I have come across this form. What should I do??
